    Characteristics of suicide in the city of Niš within the period 2000-2010

    © 2018, Inst. Sci. inf., Univ. Defence in Belgrade. All rights reserved. Background/Aim. Suicide is a significant public health problem worldwide. Numerous factors contribute to suicide. The aim of this study was to investigate the characteristics of suicide in the city of Niš in the period 2001-2010. Methods. The retrospective study consisted of 608 persons divided into three groups: suicide committers with mental disorders, somatic disorders or without registered disorders. Data on socio-demographic characteristics, previous suicide attempts, methods of suicide and presuicidal syndrome were obtained from the Police Directorate for the city of Niš, Ministry of Internal Affairs, Republic of Serbia and from the Statistical Office of the Republic of Serbia. Results. Persons with mental disorders were the most prevalent group of people who committed suicide (54.3%), followed by persons without registered disorders (31.9%) and with somatic disorders (13.8%). Persons with mental disorders who committed suicide were most often divorced, with high school education, monthly salary and with at least one previous suicide attempt. The hanging was the most frequent method of committing suicide in all investigated groups, followed by self-poisoning using drugs or liquid substances. The presuicidal syndrome was significantly more frequent among persons with mental disorders compared to persons with somatic disorders or without registered disorders others (45.8% vs. 16.7%, and 45.8% vs. 28.4% respectively p < 0.001). Conclusion. Although the persons with mental disorders are in the greatest risk of suicide, they are under medical care. In this regard, the prevention programs should be directed towards persons with severe somatic disorders and to old persons without registered disorders.

    Efficiency of IVUS guided procedures in center without previous experience

    Introduction of IVUS procedures in the routine practice is not yet achieved nor there are significant studies that dealt with examining the success of IVUS guided PCI, showed a significant advantage over conventional angiography guided PCI. On the other hand a number of smaller studies give results for the benefit of IVUS. The aim of the research is to check the effectiveness of IVUS guided PCI in the center with no previous experience in performing intravascular ultrasound diagnostics. © 2010 IEEE.

    Fractional flow reserve of intermediate lesions on collateral donor coronary arteries after myocardial infarction

    Fractional flow reserve (FFR) is the gold standard for the functional assessment of coronary arteries. The aim of this study was to evaluate the relation between angiography, QCA and FFR in borderline lesions on collateral donor coronary arteries. In addition, FFR is compared with the angiographic appearance of collaterals to infarction-related arteries and echocardiographically assessed viability of infarct related the LV wall. In 60 patients with previous IM and occluded IRA, functional assessment of borderline coronary stenosis (30-70% DS) on collaterals donor artery was performed. We have not found statistically significant differences in these parameters between groups with different angiographic appearances of collaterals and different viability of distal myocardium. However, we found higher FFR values in diabetic patients (p=0.018). Higher FFR values in diabetic patients reveal the negative effects of diabetes on collateral growth and myocardial viability.

    Predicting no-reflow phenomenon prior to primary percutaneous coronary intervention using a novel probability risk score derived from clinical and angiographic parameters

    Objective: We aimed to create a clinically usable probability risk score for prediction of no-reflow (NRF) phenomenon prior to primary percutaneous coronary intervention (PPCI). Patients and Methods: This single-center and retrospective study included 1254 patients with acute ST-segment elevation myocardial infarction (STEMI) who underwent PPCI. Patients were randomly assigned into two groups in the ratio 2:1, the derivation dataset (n=840) and validation dataset (n=414). Independent predictors of NRF were identified and combined to create a prediction model using univariate and multivariate regression analysis in the derivation dataset. The risk score was tested and validated by calculating area under the receiver operating characteristic (ROC) curves in the derivation and validation datasets, respectively. Results: Five significant, independent predictors of NRF were identified: Age ≥ 65 years (odds ratio [OR]: 2.473, 95% confidence interval [CI]: 0.389-1.484, p < 0.01), heart rate ≥ 89 bpm (odds ratio [OR]: 1.622, 95% confidence interval [CI]: 0.024-0.945, p < 0.05), Killip class ≥ II (odds ratio [OR]: 1.914, 95% confidence interval [CI]: 0.024-1.306, p < 0.01), total ischemic time ≥ 268 min (odds ratio [OR]: 2.652, 95% confidence interval [CI]: 0.493-1.565, p < 0.01), and thrombus burden G≥4 (odds ratio [OR]: 8.351, 95% confidence interval [CI]: 0.344-15.901, p < 0.01). The risk score was created combining these predictors with assigned points. The overall score ranged from 0 to 17 points. The optimal cutoff value of the risk score was 11 points (area under curve [AUC]: 0.772, 95% confidence interval [CI]: 0.729-0.815, sensitivity 71.21%, specificity 70.34%, positive predictive value 30.92%, negative predictive value 92.91%, p < 0.001). The ROC curve for the validation group showed good discriminant power. Conclusions: We developed a novel risk score based on five clinical and angiographic parameters, which might be a useful clinical tool for prediction of NRF in STEMI patients prior to PPCI with an acceptable accuracy.

    The widow maker warning sign or wellens' syndrome: A case report

    Wellens' syndrome, also known as LAD (left anterior descending) coronary T-wave syndrome, widow maker or warning sign, is a potentially unrecognized critical proximal LAD stenosis with possible fatal consequences. It can be associated with extensive acute anterior wall myocardial infarction, with left ventricular dysfunction and a lethal outcome within a few days after the onset of symptoms. It usually consists of a typical ECG finding in the precordial leads that represents a significant proximal LAD stenosis in patients with unstable angina pectoris. Although this syndrome is not indicated for PCI (the patient is usually pain-free at the time of electrocardiography registration), it is necessary to recognize the characteristic pattern and perform an emergency coronary angiography and percutaneous or surgical revascularisation of the affected blood vessel. Here we present the case report of a 47 year-old woman without previous anamnesis of coronary disease. On admission to the Coronary Care Unit she was chest pain-free and had all the indicators of Wellens' syndrome.

    Weighted Moore–Penrose generalized matrix inverse: MySQL vs. Cassandra database storage system

    © 2016, Indian Academy of Sciences. The research in this paper refers to two areas: programming and data storage (data base) for computing the weighted Moore–Penrose inverse. The main aim of this paper analysis of the execution speed of computing using PHP programming language and data store. The research shows that the speed of execution gives considerable difference between the Procedural programming and Object Oriented PHP language, on the middle layer in the three tier of the web architecture. Also, the research concerning the comparison of relation database system, MySQL and NoSQL, key value store system, ApacheCassandra, on the database layer. The CPU times are compared and discussed.
